Transfemoral Amputation Following Chronic Spinal Cord Injury: A Prosthetic Solution for Improved Balance, Seating, Dynamic Function and Body Image

چکیده
Background: Advances in the care and rehabilitation of patients with spinal cord injuries (SCI) have resulted in extended survival. As SCI patients age, chronic lower extremity ischemic complications are resulting in amputations. The literature relating to lower extremity amputation and prosthesis in SCI patients is sparse. Limb amputation may cause weight distribution imbalances, leading to back pain and increased risk of pressure ulcer formation. Lower extremity amputation challenges skin management, sitting balance, functional range and body image. Case Description and Methods: We report a systematic retrospective review of a disability adaptation equipment entrepreneur with T4 AIS B paraplegia patient who underwent right transfemoral amputation because of poor wound healing in the setting of severe peripheral vascular disease. The prosthetic prescription included a total-contact socket with a knee-flexed formed prosthesis, mirroring the opposite limb. The socket design allowed decompression of the right ischium and secured the patient in the wheel chair for seated reach and leverage in work. The patient consistently used the prosthesis over 10 years and recognized a variety of benefits including improved seating stability, functional reach, transfers, cosmesis, dressing options and body image, and documented increase in community activity. Conclusion: Lower extremity prosthesis fitting for patients with SCI and amputation can improve posture, seating, transfers, static and dynamic balance, participation in functional activities, and community reintegration. Clinical Significance: Active patients with paraplegia and new transfemoral amputation should be offered a trial of a prothesis to enhance mobility. Many of the medical and functional consequences of amputation after SCI can be prevented with careful prosthesis selection, prescription, and training patients in the use of lower extremity prosthesis. *Corresponding author: Albert Recio, Johns Hopkins School of Medicine, Physical Medicine and Rehabilitation, 801 North Broadway 560, Baltimore, MD 21205, USA, Tel: 4439239227; E-mail: [email protected] Received October 22, 2013; Accepted December 20, 2014; Published December 22, 2014 Citation: Recio AC, Felter CE, Alana ANC, Crane DA, Stiens SA (2014) TTransfemoral Amputation Following Chronic Spinal Cord Injury: A Prosthetic Solution for Improved Balance, Seating, Dynamic Function and Body Image. Elective proximal lower limb amputation in spinal cord injury patients with chronic pressure ulcers: improve quality of life, function, and shorten hospital stay. Case report.
BACKGROUND Pressure ulcers are common among patients with spinal cord injury and can be very challenging to treat. The treatment involves multidisciplinary approach and ranges from simple pressure relieve and wound dressings to a more radical treatment like proximal lower limb amputations, especially in chronic cases with potential detrimental effects to physical and mental health. AIM To rep...

Objective Spinal cord injury is a major problem for all communities that affect personal and social aspects of the patient’s life. The most common issues that spinal cord injury patients face are paralysis, muscle atrophy, pain, and spasticity. The ability to walk also may be disrupted or lost in many of the patients with spinal cord injury. Most common approaches to rehabilitation for pa...
متن کاملA Review of the Occurrence and Mechanisms of Induction of Osteoporosis Following Spinal Cord Injury
Introduction: Spinal cord injury (SCI) causes devastating injuries in patients. The main mechanisms of the pathogenesis of secondary injury include nerve degeneration, gliosis, and inflammation. Spinal cord injury induces a disorder or failure in several organs due to the vital role of the spinal cord in regulating bodily functions. Osteoporosis is a consequence of spinal cord injury that occur...

متن کاملCompound Action Potential of Isolated Spinal Cord: A Biophysical Analysis to Address Activity of Individual Fibers Following Contusion Injury
Compound action potential (CAP) of spinal cord represents valuable properties of neural fibers including excitability, rate of myelination and membrane integrity. These properties are measured using amplitude, latency and area under curve of CAPs recorded from spinal cord. Here, the isolated spinal cord was set in a double sucrose gap (DSG) chamber and its response to intracellular stimulation ...
